How to get a Hollywood Walk of Fame star
The Hollywood Chamber of Commerce founded the Hollywood Walk of Fame, HWOF (Hollywood Walk of Fame) in 1960. The HWOF is located at 6233 Hollywood Boulevard in Los Angeles, California. It's made up of 2,711 stars, which are embedded in the sidewalk on Hollywood Boulevard. Each star represents someone who contributed to motion pictures, television, radio, theater, music, or some aspect of popular culture. There are currently 1,922 active individuals on the HWOF.
You must first submit your name and address to the Hollywood Chamber of Commerce to become a member of HWOF. The Hollywood Department of Cultural Affairs then submits your information to the City of Los Angeles Department of Cultural Affairs. Once you are approved, you will receive a certificate of eligibility that includes your name, date, residence, occupation, category, and other information. You also receive a bronze plaque with your name engraved on it. This plaque is attached to a concrete base located in front of the designated Star.
You may apply to be a star if you are nominated by a HWOF member or if you make significant contributions to television, radio, TV, theatre, music or other forms of "popular culture". A committee reviews the nominations and determines whether they should be approved. If they do approve them, they send a note to the home address of the applicant stating their decision. A second letter confirms acceptance if the nominee accepts the nomination. The Hollywood Chamber of Commerce then charges $25 and forwards the money on to the City of Los Angles Department of Cultural Affairs.
The department issues an official check payable to Hollywood Chamber of commerce. This check is then sent to the Hollywood Walk of Fame Foundation. The foundation pays the applicant or organization. The Hollywood Chamber of Commerce pays half of the $12.50 fee to the artist or organization. The other half goes to the City of Los Angeles Department of Cultural Affairs.
Once the artist has received his/her star, he/she becomes eligible to be included in the HWOF. To be eligible for the HWOF, the artist must pay the Hollywood Chamber of Commerce any balance. The amount due depends upon how many years ago your first nomination was. If you were first nominated for a nomination in 1990, your owe $15,000. You would be responsible for $10,000 if you had been nominated previously in 1980.
To remove yourself from HWOF, contact the Hollywood Chamber of Commerce. They will help you pay the remaining debt. They will take care of removing your name from the list permanently.

You must be at the least 18 year old to be nominated. A person must be present at the time of receiving a star. After receiving a Star, one can still be Nominated for another star after ten Years.
What happens if I die before my star gets put up?
You can be nominated by your family members to receive a star. The Hollywood Chamber of Commerce will place an honorary marker in front of the star if there are no living relatives.
