How to get a Hollywood Walk of Fame star
The Hollywood Chamber of Commerce founded the Hollywood Walk of Fame, HWOF (Hollywood Walk of Fame) in 1960. The HWOF is located at 6233 Hollywood Boulevard in Los Angeles, California. It has 2,711 stars embedded in the Hollywood Boulevard sidewalk. Each star represents an individual who contributed to popular culture, such as television, radio or theater. There are currently 1,922 active individuals on the HWOF.
You must first submit your name and address to the Hollywood Chamber of Commerce to become a member of HWOF. The Hollywood Department of Cultural Affairs then submits your information to the City of Los Angeles Department of Cultural Affairs. Once you have been approved, you will be given a certificate that shows your name and details, including date of birth, place of residence, occupation, and category. A bronze plaque with the name of your choice is included. This plaque will be attached to a base of concrete in front the designated star.
You may apply to be a star if you are nominated by a HWOF member or if you make significant contributions to television, radio, TV, theatre, music or other forms of "popular culture". A committee evaluates all nominations and then decides whether to approve them. If they do, they will send a letter explaining their decision to the applicant at his/her home address. If the person accepts, they will be sent a second letter to confirm their acceptance. The Hollywood Chamber of commerce then sends $25 to the City of Los Angles Department of Cultural Affairs.

The amount due depends upon how many years ago your first nomination was. If you were first honoured in 1990, then you would owe $15,000. You'd owe $10,000 if you were previously nominated in 1990.
You can remove yourself from HWOF by contacting the Hollywood Chamber of Commerce to pay the rest of your debt. They will take care of removing your name from the list permanently.
